Students registering for Athletics must have a current WIAA Medical Eligibility Form on file before the student is authorized to participate in practice/tryouts. To ensure clearance for participation, physicals/alternate year cards must be completed and submitted prior to the first day of practice/tryouts. Completed physicals or Alternate Year Cards can be dropped off in the main office, uploaded into the online registration form, or scanned/emailed to Tremperathletics@kusd.edu
Exam Dates of March 31, 2025 or earlier = Student needs an updated exam with WIAA Medical Eligibility Form
Exam Dates between April 1, 2025 and March 31, 2026 = Student needs an Alternate Year Card
Exam Dates after April 1, 2026 = No Alternate Year Card needed
